Cancellation Policy:
A 100% refund of course fees will be provided if the MSMS Foundation is notified by September 26, 2016. The $35 registration fee is non-refundable. No refunds will be given after September 26, 2016; however substitutions are always welcome.

CME Credits

Statement of Accreditation:
The Michigan State Medical Society (MSMS) is accredited by the Accreditation Council for Continuing Medical Education (ACCME) to provide continuing medical education for physicians.

AMA Credit Designation Statement
The MSMS designates this live activity for a maximum of 27.25 AMA PRA Category 1 Credit(s)™. Physicians should claim only the credit commensurate with the extent of their participation in the activity.

Nurses
The ACCME is approved by the Board of Nursing as an acceptable provider of continuing education for license renewal or relicensure.

AAFP Credit
Application for CME credit has been filed with the American Academy of Family Physicians. Determination of credit is pending.
